EMT Skills Checklist Templates

EMT Skills Checklist Templates

Score every skill with these free EMT skills checklist templates, whether it's organized by station or by competency domain, before you sign off training or recertification. Each template flags the critical steps, then records the evaluator, the candidate, and a result of pass, needs remediation, or competent.

Personalize the chosen design, then generate a signed checklist for every candidate at the end of each exam or assessment. Import the list of candidates from a spreadsheet or one of Certifier's integrations, email each credential, and host a verification page an EMS agency, training provider, or medical director can check.

EMT Skills Checklist Templates FAQs

Nothing stops you from issuing the certificate before the checklist is done, so it's a process you manage yourself. On Advanced, you can enforce it with Pathways: set the checklist as a criteria credential and the certification as the completion credential, so Certifier issues the certification automatically only once the checklist, and anything else you require, has been issued to that recipient.

Certifier's EMT checklists work either way since the layout is fields you control, not a fixed structure. Organize by station if your program runs candidates through set skill stations, airway, C-spine, patient assessment, and evaluates each stop separately. Or organize by competency domain instead if you're scoring broader skill categories across an entire encounter rather than station by station.

Retention periods for EMT records are usually set by your state EMS office, your employer's policy, or your accrediting body, not by Certifier. Certifier keeps issued checklists accessible in your account indefinitely, so the document itself won't disappear.

Reuse the same template across cohorts. Certifier separates the template's design from the per-recipient fields, so you just fill in each cohort's names, dates, and evaluators when you issue. There's no need to duplicate or rebuild the template itself for every new group.
