First Aid & CPR Checklist Templates
Confirm every First Aid and CPR skill was demonstrated before you certify a trainee with these free checklist templates. Each template lists the required skills or steps, then records who demonstrated them, on what date, and whether they passed.
Personalize the chosen design, then generate a signed checklist for every trainee at the end of each session. Import the list of trainees from a spreadsheet or one of Certifier's integrations, email each record, and host a verification page an employer or first aid training provider can check.

First Aid & CPR Checklist Templates FAQs
Certifier's document template collection has separate First Aid Skills Checklist and CPR Skills Checklist designs, so start with whichever matches your class. There isn't a dedicated combined-course template. For a course that covers both, use one of the two as your base and add the fields you need to cover the other skill set or issue both checklists to each participant.
You can batch-issue once you have each participant's results, generating every signed checklist from a spreadsheet or import in one pass instead of creating them one by one. Each participant still receives their own individual, verifiable document.
The expiration lives on the certification, not the checklist. On Professional and Advanced plans, open the certification's credential template, go to Advanced Settings, and turn on expiration with whatever duration your program follows, two years for example.
Certifier then automatically emails the recipient a reminder two weeks before it expires and again on the expiration day, so reassessment timing takes care of itself without you tracking dates by hand. The checklist itself doesn't carry an expiration date, so you need to reissue a fresh one alongside the renewed certification when reassessment happens.
Yes. Edit the fields to fit a refresher, trimming steps a returning participant doesn't need to repeat and noting it as a recertification rather than an initial assessment. You're not locked into the checklist's original field list.
